Ear, Eye, and Dental Form
Proof of Residence
If you reside in an apartment the Original lease is needed. If you are a homeowner or renting a house please submit a Recent utility bill, i.e., Water, Electric or Gas. Should you reside with a family member or friend please provide their proof of residency along with the affidavit of residence signed by both parties and notarized.
